Welcome! This web resource will help you as partners to develop your Health, Social Care and Well-being strategies. This site assists local partnerships to develop their Health Social Care & Well-being strategies and to measure progress on partnership outcomes.

Discussions between Health Social Care & Well-being (HSCWbHealth, social care and well-being) partnerships in Wales, the Welsh Local Government Association (WLGAWelsh Local Government Association) and the Local Government Data Unit ~ Wales (the Data Unit), identified a need to produce a set of partnership outcome measures that would be consistent across Wales.

These measures will enable comparisons to be made between areas and also investigations of local conditions. These outcomes are partnership outcomes, that is requiring significant contributions from two or more partner organisations.

This site has been designed in four parts:

• Getting started: some basic information about how to use the website;
• Knowledge library: documents containing supporting information;
• Access to data: links to the data and to details about the measures;
• Information exchange: where good ideas and lessons learned can be recorded;
